Chorus: You're going up half-steps from the C all the way to Em. The C#m7b5 has this shape: d|------------------| A|--5---------------| F|--4---------------| C|--5---------------| G|--4---------------| D|------------------| Then your D will be a bar chord with 5th fret fifth string as bass note, then the D#m7b5 is just the C#m7b5 shape above moved up two frets (6th fret bass note) so your Em will have the 7th fret as your bass note.
